Admissions Criteria

Admissions criteria for palliative medicine

• Individuals residing in the Hospice & Palliative Care Charlotte Region service area (Mecklenburg, Iredell, Cabarrus, Union, Lincoln, Gaston, Cleveland, and Catawba counties)
• Individuals with a life-limiting illness who desire care with a focus on comfort, pain and symptom management, and other supportive interventions
• A palliative medicine referral from a physician

Admissions criteria for hospice care

• Individuals residing in the Hospice & Palliative Care Charlotte Region service area (Mecklenburg, Iredell, Cabarrus, Union, Lincoln, Gaston, Cleveland, and Catawba counties)
• Patients referred and followed by their physician or healthcare provider, and who have a life expectancy of six months or less.  Referrals may also be made by patients or family members
• Individuals who live in assisted living residences or skilled nursing facilities and require end-of-life care
