Kids Path® Program Criteria for Admission

1. A child is in need of a specialized home care approach as a result of a progressive and potentially life limiting illness or condition.
2. The child resides in our service area (Cabarrus, Catawba, Cleveland, Gaston, Iredell, Lincoln, Mecklenburg, and Union counties).
3. The attending physician agrees to direct the medical plan of care (for home health or hospice services), and participates as a member of the Kids Path's care team.
4. The intial plan of care defines which care option will be appropriate.  All referrals will be evaluated for eligibility and will be accepted or referred appropriately.
