What is our Veteran Care program?

We understand that our Veterans have unique needs and we proudly serve them...

One out of every four individuals who die in the United States is a Veteran. And Veterans often have unique needs at end-of-life.

We have partnered with the “We Honor Veterans” program, a joint initiative of the National Hospice and Palliative Care Organization (NHPCO) and the Department of Veteran Affairs. Comprised of five levels, VIA Health Partners has achieved Partner Level Five, the highest recognition of this esteemed program. This distinction reflects our continuing commitment and dedication to providing Veteran-centric education to staff and volunteers, developing and strengthening relationships with VA medical centers and other Veteran organizations, and most importantly, ensuring Veterans have access to quality end-of-life care.

VIA Health Partners staff members receive ongoing training throughout the year to address the unique healthcare needs of Veterans, both physical and emotional, including the psychological toll of war and how it impacts their end-of-life journey. In some cases, Veteran patients are paired with volunteers who have also served their country and share a unique and honorable bond. Each Veteran under our care receives acknowledgement for their service in a small, private observance that concludes with the presentation of a commemorative pin. The pin is our way of saying "thank you" to the brave men and women who have protected the freedom of our country.
